CareCam Health Systems (CCHS) has teamed-up with the Independence Blue Cross Center for Health Care Innovation to develop several programs to improve the care of people with chronic health conditions. Also as part of the deal, Independence Blue Cross has become a minority investor in CCHS, which chronic care management software platform promotes patient adherence to prescribed therapy and seamless coordinated care.
CCHS’s vHealth patient-driven software combines the power of behavioral science, mobile technology and video to enable scalable, successful chronic care management. The solution drives improved health outcomes for health plan members, while reducing costs across all books of business.
Launched in February 2014, the Independence Blue Cross Center for Health Care Innovation is home to Blue Cross’ widening range of innovation initiatives, including strategic investments in startups and venture funds, national research partnerships with leading universities, a regional task force on health care innovation, and its support of DreamIt Health Philadelphia, the region’s first health care business accelerator.
