Heart rate accuracy in consumer wearables has long been the weak link between lifestyle data and clinical utility. Apple is directly targeting that gap with the Ultra 4, citing a study of over 1,000 participants in which its new optical and electrical sensor system outperformed competing devices on accuracy. That’s a meaningful claim, and one the company says is backed by rigorous methodology, though independent peer review will determine how much weight clinicians should give it.
What’s actually new in the hardware
The S11 chip powers what Apple calls the Health Sensing System, which measures heart rate every five seconds throughout the day, up to 24 times more frequently for heart rate variability (HRV), and separates HRV into two clinically relevant variants: recovery HRV for day-to-day stress and recovery signals, and overall HRV for broader cardiovascular context. Battery life has also extended meaningfully, reaching 50 hours in standard use, 84 hours in Low Power Mode, and 45 hours during continuous outdoor GPS tracking. Fifteen minutes of charging adds up to 18 hours of use.
The readiness score and its clinical roots
The headline feature is readiness, a 0-10 daily score that combines recent activity, training load, vitals, and sleep data into a single recommendation: Recover, Pace Yourself, Ready, or Go For It. Apple says the algorithm was developed using data from its Heart and Movement Study, in collaboration with exercise scientists and physicians. That’s not a trivial foundation. Still, a score distilling complex physiology into one number will always carry interpretive risk, and clinicians working with athletes should understand its inputs before treating it as diagnostic.
What’s coming later this year
A redesigned Health app for iPhone will include a Longevity tab with a Health Age feature, benchmarking a user’s metrics against age-matched norms. Audio Intelligence features, built on the S11 chip, will help users capture key conversational moments and respond to ambient sound alerts. Siri on watchOS 27 also gains Apple Intelligence capabilities for contextual, on-wrist assistance.
