Population health management (PHM) solution provider ZeOmega is acquiring HealthUnity Corporation, provider of interoperability solutions that enable providers, payers and public health organizations to streamline the secure exchange of patient data across disparate systems.
HealthUnity’s solutions have already been deployed at several integrated delivery networks (IDNs), health systems, group practices and state collaboratives. On the other hand, ZeOmega’s population health management platform Jiva delivers analytics, workflow, evidence-based content and patient engagement capabilities for organizations seeking to transition to value-based care. Combining the two companies’ capabilities will create a powerful deployable PHM infrastructure that payers and providers alike can use to drive value-based care, not only within their organization, but also within their communities.
