Thresholds and Wellframe and are teaming-up to provide members in its Veterans Project with Wellframe’s mobile technology to help them better navigate the healthcare system and “gain control over their own care.”
Wellframe enables health plans and healthcare providers to amplify existing care resources to improve patient engagement and better manage care, while Thresholds provides recovery services for persons with mental illnesses in the Chicago area, and delivers such practices as homeless outreach, housing services, supported employment, peer-driven supports, substance abuse treatment, integrated physical and mental health services, and trauma-based therapies. Their veterans-specific program, the Veterans Project, is a leader in community-based social services tailored specifically for veterans facing mental health challenges.

Combined with the valuable resources and tools available through Wellframe’s mobile app, veterans can overcome barriers to care, leading to long-term recovery.
