Care at Hand raises $560K for its healthcare analytics platform

Care at Hand has raised $560,000 for its healthcare analytics platform from PCG Public Partnerships. The company provides an easy way for direct care (non-clinical) workers to export valuable “red flag” health related events to Case Managers, helping predict and prevent hospitalizations along the way. Its mobile app is unique to the Home and Community Based Services population.

A division of Public Consulting Group (PCG), PPL helps states, counties and local public agencies implement a participant-directed service model, which allows participants to make choices about which services they receive, how they are delivered and by whom, within their budgets.

Care at Hand’s social impact was recognized as a finalist in this year’s MBA Impact Investing Network & Training (MIINT) competition.

A recently accepted study in the journal Perspectives in Health Information Management shows that this technology can predict hospitalization risk using non-medical workers, as well as, an AHRQ study showed a 39.6% reduction in readmissions and net savings of $2.57 for every $1.00 invested in technology.
