Qualcomm Life launched the HealthyCircles Mobile app, a secure, medical-grade mobile care solution that provides on-the-go care experience for consumers via their mobile devices. An extension of Qualcomm Life’s web-based HealthyCircles Care Coordination Platform, the application offers personalized care, patient activation and behavior modification for chronic care patients.
HealthyCircles Mobile is an enterprise private-label chronic care management app that supports iOS and Android devices, enabling access to vital health information and secure communications for care team coordination. It’s built on Qualcomm Life’s 2net Mobile core, an FDA-listed Class I MDDS secure gateway that can capture biometric data from a wide array of medical devices, allowing patients to visualize their own biometric data in color coded trackers. And with HealthyCircles Mobile, patients have access to these personalized care features in the palm of their hands and can take a more active role to adhere and engage in their health.
“HealthyCircles Mobile is an adaptive, personalized mobile care experience for patients who need it the most: people with chronic conditions and those transitioning from hospital to the home,” said James Mault, MD F.A.C.S., vice president and CMO, Qualcomm Life. “Our goal is to provide a frictionless platform that will guide healthy behaviors and motivate patients to engage in their own self-care.”
