Qualcomm Life announced new partners for the 2net Device Connectivity Platform and HealthyCircles Care Coordination Platform during its annual Connect 2015 conference.

Davita HealthCare Partners, one of the largest medical groups and physician network operators, will integrate Qualcomm Life’s solutions into a coordinated care program for patients with heart failure (HF). Patients will be equipped with a home blood pressure monitor, a weight scale and a tablet powered by 2net Mobile to seamlessly collect and transmit the biometric data to the cloud. Also, patients will be able to benefit from the HealthyCircles Platform to monitor their health status, receive relevant educational content and targeted care management interventions. The initial program will roll out in the fourth quarter of the year.

On the other hand, P2Link will work with Qualcomm Life in the United States and Europe to deploy remote monitoring medical device kits powered by the 2net Hub and Platform for patients suffering from heart failure and chronic obstructive pulmonary disease (COPD), with the first patient enrollment expected to begin in the United States in Q4 2015. P2Link is looking to reduce the cost of care for HF and COPD patients by 10 to 15 percent; its first implementation of 2net will be with Emerson Hospital in Concord, Massachusetts.

“With our growing ecosystem of more than 500 companies, we are collaborating to develop new care models that deliver near real-time informed care that’s both adaptive and patient-centered,” said Rick Valencia, senior vice president and general manager, Qualcomm Life. “We are thrilled to be working with HealthCare Partners and P2Link to deliver the right care, at the right time, in the right location to the patients who need it the most.”
