Uncovr raises $7M to automate surgical documentation with AI

The startup uses video from the operating room to generate clinical records automatically, tackling one of healthcare's most persistent paperwork problems

Surgeons spend a significant portion of their working lives writing reports about procedures they’ve just performed, relying on memory rather than objective data. Uncovr, a surgical AI startup founded in 2025, wants to change that by turning operating room video into structured clinical records automatically. The company has raised $7 million in seed funding to push that vision forward.

The round was led by Index Ventures, with backing from Seedcamp, Frst, No Label Ventures, and Entrepreneurs First. A number of experienced operators also joined the round, including Jean Nehme, founder of Digital Surgery; Othman Laraki, CEO of Color Health; and Charlie Songhurst, a Meta board member. That combination of venture capital and domain expertise suggests investors see real commercial potential here, not just a technical curiosity.

Uncovr was founded by Ines Iraki, Johann Diep, and Professor Eric Vibert. The company is already working with hospitals in the United States and Europe and says its deployment pipeline covers more than 400 operating rooms. Thousands of hours of surgical footage have been processed through the platform so far.

Why surgical documentation is such a big problem

Operative reports are among the most consequential documents in healthcare. They form the official clinical and legal record of a surgery, and hospitals depend on them for patient care continuity, insurance reimbursement, compliance, and future treatment decisions. Getting them wrong, or leaving them incomplete, has real consequences.

The problem is that most of these reports are still written manually, often after the procedure has ended, from memory. That creates obvious risks around accuracy. Meanwhile, robotic and minimally invasive surgery has made video capture increasingly common in operating rooms, yet that footage has largely gone unused for documentation purposes.

How Uncovr’s platform works

Uncovr’s system analyses surgical and endoscopic video recordings to build structured procedural records in real time. By the time a surgeon leaves the operating room, a draft report and procedural coding recommendations are ready for review. Nothing is submitted without human sign-off.

The core idea is to ground documentation in what actually happened during a procedure, rather than what a surgeon recalls happening. According to the company, this approach can improve:

  • Clinical accuracy in operative reports
  • Coding quality and reimbursement integrity
  • Continuity of care across treating teams
  • Searchable records for quality assurance, compliance, and research

Early hospital deployments have also flagged reimbursement gaps that traditional review processes missed entirely, pointing to a financial case for adoption that goes beyond the obvious administrative benefits.

The bigger picture: surgical data as an asset

Beyond documentation, Uncovr is positioning itself to turn surgical procedures into structured clinical datasets. Those datasets could, over time, support medical research, hospital operations, and the next generation of AI tools built specifically for surgery.

This connects to a wider trend in health tech. As AI models become more capable, the value of high-quality, structured medical data is rising sharply. Surgical video is one of the richest and most underused sources of clinical data available, and companies that can process it at scale will have a significant advantage in building specialized AI for the operating room.

What the funding will be used for

The $7 million will go toward three main areas:

  • Continued development of Uncovr’s AI models for surgical documentation and workflow analysis
  • Expanding the number of hospital deployments
  • Broader product development across the platform

For a company founded just last year, the combination of an Index Ventures lead, a strong hospital pipeline, and a problem that hospitals are clearly willing to pay to solve makes Uncovr one of the more credible early-stage health AI bets of 2026.
