An app launches to support people living with heart failure

The Heart Failure Society of America (HFSA) is launching the Heart Failure Health Storylines app and platform, to support self-care of people living with heart failure, and congestive heart failure (CHF).

Developed in partnership with patient intelligence and health solutions provider Self Care Catalysts, the application makes it easy for patients to track their symptoms, sodium intake, vital sign, moods, as well as sync with other apps and devices such as Fitbit and Jawbone. It also includes medication reminders, while creating so called “My Storylines” for each user that enables them to be more informed about their health. The same interface should provide for a more productive conversation with a care team.

“It is essential that both patients and their family understand what heart failure is, what the symptoms are, what they should do if symptoms change and how their providers treat this disease,” Michele Blair, Heart Failure Society of America’s CEO, said in a statement. “This is what the Heart Failure Health Storylines App will allow patients and their families to do — to stay on top of their symptoms through easy tracking on an app, as well as monitor other important aspects of their health, like their vital signs, medications, and sodium intake.”

Heart Failure Health Storylines is free for all users and is available on the web, the App Store and on the Google Play Store.
